Bright Beginnings Preschool Registration/Enrollment Forms:

Once our director has confirmed your child's placement for the 2024-2025 school year, the following payments and forms must be submitted.

​

1. Submit the $60 non-refundable registration fee ($100 for 2+ children).

​

2. Submit one month's tuition for the program in which your child is registered (non-refundable, tuition will not be due again until October) :

​

• Parent's Morning Out (PMO) Wednesday Class: $65

• Parent's Morning Out (PMO) Mon/Thurs, Tues/Fri: $130

• 2 Day (Tues/Thur AM) Program: $140

• 3 Day (Mon/Wed/Fri AM) Program: $160

• 4 Day (Mon/Tues/Thu/Fri PM) Program: $205

• 5 Day (Mon-Fri AM) Program: $205

• Kinder Prep (Mon-Fri PM) Program: $220

3. Submit a copy of your child's most recent Immunization and Physical Forms

• ​​Please see your child's physician to obtain a signed & dated copy of their immunization form and physical. Forms can either be faxed to Bright Beginnings Preschool (Fax #: (515) 964-4392) OR e-mail a PDF file to brightbeginnings@ankenyfirst.org

4. Fill out and submit the following PDF files of the following Enrollment Forms: 

 (Click download, then fill out and save. If you fill out and then download, it will not save your work!)

• Student Intake & Consent Form

• Emergency Medical Consent Form

• Child & Family Information Form
